We are currently seeking an experienced and dedicated Director of Social Services to lead our social services team and ensure the well-being of our residents.

Position Summary:
The Director of Social Services will oversee and manage the social services department within our Skilled Nursing Facility. This is a leadership position responsible for developing, implementing, and supervising social service programs for our long-term care and short-term rehabilitation residents.

Responsibilities:

• Manage all aspects of the Social Services department, ensuring compliance with federal, state, and local regulations.

• Supervise, guide, and support Social Services staff to maintain high standards of care.

• Conduct assessments and develop care plans to address residents' physical, emotional, and social needs.

• Implement therapeutic programs and interventions for residents.

• Collaborate with the interdisciplinary care team to provide person-centered care to residents.

• Advocate for residents' rights and serve as a liaison between residents, families, and staff.

• Ensure accurate documentation and maintain records according to regulatory standards.

• Provide crisis intervention, counseling, and support for residents and their families.

• Participate in care conferences and interdisciplinary meetings related to resident care.

Qualifications:

• Bachelor's or master's degree in social work.

• Current state licensure as a Licensed Social Worker (LSW) or higher.

• Minimum of 3 years of experience in social services, including at least 2 years in a long-term care (LTC) or short-term rehabilitation (STR) setting.

• Strong knowledge of regulations governing skilled nursing, LTC, and rehab services.

• Excellent communication, organizational, and leadership skills.

• Compassionate, patient-centered approach to care.

• Ability to work independently and as part of an interdisciplinary team.

• Proficiency in electronic medical records (EMR) and documentation systems.
