Job DescriptionJob DescriptionGeneral DescriptionProvide training in independent living skills, including how to be responsible tenants and maintain psychiatric stability. Qualifications and CompetenciesBachelor’s degree in social work or equivalent degree, Master’s Degree preferred.Minimum of two years’ experience in the Human Service field.Substance use disorder/treatment experience preferredDemonstrates organizational and communication skills which would enable effective interaction with agency staff, consumers/clients/families and the community.Critical thinking and sound judgement skills are necessary. Duties and ResponsibilitiesConducts regular visits to assigned consumers and sites (minimum monthly) to monitor needs and safety.Ensures fire safety equipment is installed, functional, and understood by consumers; provides emergency preparedness education.Teaches and supports independent living skills to promote community stability and prevent homelessness.Assists consumers in accessing community resources and neighborhood services.Serves as liaison among consumers, landlords, treatment teams, and Health Home to coordinate care.Develops and supports Individualized Support Plans, including follow-through on mental health, medical, vocational, and community-based goals.Maintains compliance with mandated treatments and legal requirements, completes documentation timely, and participates in supervision and required meetings.Accepts and follows through on other duties and responsibilities as assigned. ScheduleSupportive Housing: Mondays-Fridays 9am-5pmApartment Treatment Program: Tuesdays-SaturdaysSundays-ThursdaysMinimum one late shift weekly (ending at 6pm, latest 7pm) Physical Demands & Work ConditionsHeavy travel to various apartment/site locations or other office locationsProvides transportation to clients in either personal vehicle or agency vehicleMay be responsible to physically assist clients move or transfer apartmentsAssist and/or demonstrate cleaning techniques to clients to improve independent living skills.Lifting objects, items, charts, files to and/or from various site locations.
