Licensed Practical Nurse (LPN)Title: Licensed Practical Nurse (LPN)Department: NursingShift: 1st ShiftDescriptionSt. Coletta's of Illinois operates an Intermediate Care Facility (ICF) for individuals with developmental disabilities. The Intermediate Care Facility allows for adults with intellectual and developmental disabilities to live independently while still receiving support twenty-four hours a day, seven days a week.The nursing department is seeking an LPN to provide medical treatment and personal care to individuals with intellectual and physical disabilities that are enrolled in the agency's programs. Shift: 7:00 AM - 3:30 PM Payrate: $33.45 hourly Site: Tinley Park, IL / Country Club Hills, IL ResponsibilitiesAdministers nursing care plan to assigned individuals.Administers specified medications, orally or by subcutaneous or intramuscular injection, and notes time and amount on the individual's chart.Monitors and records vital signs.Collects laboratory specimens, such as urine, blood and sputum.Assembles and used such equipment as catheters, tracheotomy tubes, and oxygen supplies.Sterilizes equipment and supplies.Monitors health status of individuals on assigned caseload.Performs clerical duties and maintains medical records in accordance with state, federal and accreditation requirements.Prepares or examines food for prescribed diets.Assists in the maintenance of a safe physical environment for individuals served.Observes individuals and reports adverse reactions to medication or treatment to supervisor.Dresses wounds, gives enemas, douches and provides other treatments as required.Ensures all medication orders are promptly and accurately communicated to the pharmacy, to the appropriate sites and to the staff administering the medications.Ensures all prescribed (including over-the-counter) medications are at the site and are accurately administered.Accurately completes Medication Administration Record.QualificationsIllinois License required.One-year experience with medication administration.Pharmacology certificate.Prior experience with intellectually and/or physically disabled individuals.Valid Driver's License with acceptable driving record required.Must pass a physical, drug screening and MVR background.We offer the following benefits: health, dental, life, LTD, 403b retirement plan, paid vacation, personal days, sick days and 10 paid Holidays.Founded in 1949 by the Sisters of St. Francis of Assisi, St. Coletta's of Illinois provides a lifelong continuum of care beginning in early childhood.
