Remote Spanish-Speaking RN Case Manager

Alignment Health is seeking a Remote RN Case Manager SNP (California RN license required, non-compact) to manage care for members with complex, chronic needs. You will coordinate with PCPs, hospitalists, and care teams, conduct assessments, close gaps in care, and support self-management. Must have 2+ years clinical experience and 1 year in case management. Remote work from CA with Pacific Time schedule; opportunities for growth and collaboration across the Alignment Health team.
