Key Highlights Weekly pay: $2,295–$2,304 per week Guaranteed hours: 40 per week Contract duration: 17 weeks Shift: 8-hour shifts Start date: 09/07/2026 Work setting: Skilled Nursing Facility (travel assignment) Responsibilities Assess home health and hospice patients in stable and emergent situations to identify physical, psychosocial, and environmental needs; document findings. Implement, develop, document, and evaluate the Plan of Care to ensure quality and continuity of care. Obtain data on physical, psychological, social, spiritual, and cultural factors that may influence patient/family health status and incorporate into the Plan of Care. Maintain up-to-date patient records with visits, contacts, problems, plans, actions, and goals; ensure changes are reflected as they occur. Provide care utilizing infection control measures; educate patients and/or families regarding care, medications, home safety, and self-care practices. Supervise and provide clinical directions to the HHA/STNA/LVN to ensure quality and continuity of service. Assess and reassess pain; utilize appropriate pain management techniques and educate on pain control. Arrange referrals to appropriate agencies when needed; monitor cases for third-party payer compliance. Participate in interdisciplinary team meetings and perform on-call responsibilities as assigned.
