We are recruiting for the following position:Sea Mar is a mandatory COVID-19 and flu vaccine organizationSea Mar is seeking a Care Coordinator to support patients with chronic conditions and behavioral health needs. As a key member of the Clinical Care Team, you will work at point of care to identify patient needs, close care gaps, and coordinate services that improve patient outcomes and quality of care.Key ResponsibilitiesParticipate in daily huddles and identify gaps in careProvide brief interventions and support chronic disease managementCoordinate care transitions and follow-up after ER/hospital visitsLink patients to behavioral health, dental, and community resourcesTrack and document patient progress in the EHRSupport quality improvement initiatives and clinical performance measuresAssist with 340B medication assistance programs (site-dependent)QualificationsMedical Assistant training + 1+ year experience in community health or equivalentStrong patient engagement, communication, and problem-solving skillsExperience with chronic disease and behavioral health preferredBilingual English/Spanish strongly preferredMust obtain CPR within 90 days of hireValid WA driver’s license and proof of insurance requiredCOVID-19 vaccination requiredSea Mar is an Equal Opportunity Employer and offers a competitive benefits package.Starting Wage: $22.54 per hourWork Schedule: Monday – Friday (Can be edited based on need)Hourly - Hourly Plan, 22.54 USD HourlyWhat We Offer:Sea Mar offers talented and motivated people the opportunity to work in a dynamic and growing community health organization.
