In This Role You Will:

1. Maintains a secure, stimulating, and comfortable environment for residents with a dementia diagnosis, Alzheimer’s disease or other cognitive challenges.
2. Conducts individual resident history, preference assessments, and develops individualized service and program plans.
3. Coordinates and implements the daily life enrichment, spiritual, wellness and other programs for the Memory Care unit. Delegates program facilitation responsibilities to others, as needed.
4. Improves the memory care program through research, program development, and refinement, incorporating feedback from the team members and resident’s families.
5. Complies with the budget for the department, including providing suggestions and completing any required budgetary reports.
6. Maintains a collaborative and productive relationship with internal and external customers.


7. Collaborates with nursing staff to ensure health care needs, medication administration and other care and services are available and delivered promptly and in adherence with established policies and procedures and state and federal regulations.
8. Performs supervisory functions:


a. Participates in the hiring, onboarding, and orientation process for new employees.
b. Promptly and thoroughly addresses employee disciplinary action and terminations in coordination with human resources.
c. Provides training, coaching and supervision to employes.
d. Provides timely and accurate performance management evaluations and participates in goal setting for the employee, in accordance with the organization’s established policies and procedures.
e. Prepares, implements, and monitors staffing schedules and work assignments.
f. Administers department and organizational policies and procedures in a fair and consistent manner.
g. Completes the payroll functions in a timely and accurate manner.

9. Performs other duties as assigned.

Shift Details:

Full-time Monday through Friday

Qualifications:

• Required: a bachelor’s degree in healthcare related field and two to four years of experience caring for individuals with dementia or cognitive challenges and developing and facilitating life enrichment and other programs.
• Preferred: Certification and/or training in dementia care.
