Registered Nurse – Home Health
Schedule: Monday–Friday, 8:00 AM–5:00 PM
Guaranteed Hours: 40 hours/week
On-Call: Saturday/Sunday SOC/ROCs only
Location: Florence, Darlington, Williamsburg, Marion, Dillon & Marlboro Counties, SC
Requirements
Active SC RN license; Compact licenses accepted
Associate or Bachelor’s Degree in Nursing or related field
Home Health experience
HCHB and OASIS experience
IV experience required
Case Management certification preferred
Ability to assess complex medical information and make sound clinical decisions
Key Responsibilities
Perform comprehensive assessments and develop individualized care plans
Coordinate care with physicians, social workers, discharge planners, and other healthcare professionals
Manage care transitions and ensure appropriate levels of care
Support patient compliance with treatment plans and coordinate non-medical resources
Monitor clinical, functional, and fiscal outcomes
Identify opportunities for health promotion and illness prevention
Advocate for patients while maintaining privacy and confidentiality
Mileage Reimbursement: $0.49/mile
Productivity: 30 points/week (6 points/day)
