You'll collaborate closely with physicians, nursing, and therapy teams to deliver individualized, patient-centered care.###### **Key Responsibilities*** Perform **evaluations within 24 hours (or 1 business day)** of physician referral* Develop and implement **individualized treatment plans** with physician approval* Provide therapy services in accordance with treatment plans* Monitor patient progress and adjust care as needed* Maintain accurate clinical documentation (evaluations, progress notes, discharge planning)* Participate in **care plan meetings, utilization review, and interdisciplinary conferences*** Educate residents, families, and staff on therapy programs and techniques* Supervise **SLPAs and Clinical Fellows (CFY)** as applicable* Conduct **staff training and in-service education*** Ensure compliance with all regulatory and facility standards###### **Qualifications*** Master's or Doctoral degree in **Speech-Language Pathology*** Active, unencumbered **SLP license in South Carolina (required)*** **CCC-SLP preferred*** Experience in a **skilled nursing or rehab setting preferred*** Knowledge of **speech, language, cognitive, and swallowing disorders*** Experience with **PointClickCare (PCC) and Casamba preferred*** Strong clinical, communication, and documentation skills* Ability to work both independently and as part of a team###At Chandler Creek, you'll join a compassionate and collaborative team where your expertise is valued and your growth is supported.
