Registered Nurse - Home Care

Role The Registered Nurse provides and oversees nursing care in patients' homes, ensuring compliance with laws, regulations, and agency policies. Responsibilities Deliver high-quality clinical services within scope of practice and infection control standards Coordinate care with healthcare team from admission to discharge Conduct clinical nursing assessments following federal/state requirements and payer needs Verify patient eligibility and medical necessity for services Develop and update individualized care plans in collaboration with community providers Adjust care plans based on patient needs with physician approval Qualifications & Skills Current unrestricted RN license in practice state Reliable transportation, valid driver’s license, and vehicle insurance CPR certification Ability to serve diverse home environments and clients of various backgrounds Preferred: 1+ years of RN experience, independent work capability, strong communication and organizational skills #J-18808-Ljbffr
