Signify Health is seeking a per diem In-Home Nurse Practitioner or Physician Assistant to perform comprehensive health evaluations in members’ homes. You will review medical histories, medications, and social factors; complete focused physical exams; and document findings in our digital tools to support value-based care. Partnering with interdisciplinary teams, you’ll identify care gaps, provide education, and recommend follow-up.
