Nurse Practitioner/Physician Assistant | Rehab Admissions Full Time Green Bay, WI, US Primary Care Nurse Practitioner / Physician Assistant – Adult & Geriatric Care Rehab Hospital Admissions | Green Bay Area 0.4 FTE | Weekends Only (Saturday & Sunday) Join a mission-driven clinical team where your expertise directly improves the lives of adult and geriatric patients in an inpatient rehabilitation setting, with additional support for psychiatric admissions. Evaluate patients, establish diagnoses, prescribe medications, and perform diagnostic and therapeutic procedures within scope of practice. Develop individualized treatment plans in collaboration with physicians and interdisciplinary care teams. Coordinate care with specialty providers, including wound care, psychiatry, and other consulting services. Communicate changes in patient condition and treatment plans with patients, families, facility staff, and healthcare team members. Maintain timely, accurate, and compliant documentation. Build strong relationships with patients, families, and facility staff to support exceptional care. Perform other duties as assigned. Qualifications Required Current Nurse Practitioner or Physician Assistant license. Active DEA registration. BLS certification. Strong clinical assessment and diagnostic skills. Ability to work independently while collaborating effectively with interdisciplinary teams. Commitment to compassionate, patient-centered care. Preferred Leadership experience or demonstrated leadership skills. Strong organizational and time-management abilities. Positive, solution-oriented attitude. Experience working with adult and geriatric populations.
