Pharmacist Position

Job Specific Competencies

1. Assures that the clinic maintains standards of pharmacy practice equal to or above those accepted by the profession and the law. 2. Performs the duties of a pharmacist, including dispensing of prescriptions, counseling of patients and medication therapy management. 3. Utilizes pharmacy software and technology systems effectively, including competency surrounding patient safety measures built into such systems. 4. Participates in the process/procedures of clinical pharmacist programs to enhance patient outcomes and optimize pharmacist knowledge and impact, including the use of prescriptive protocols and therapeutic substitution policies. 5. Collaborates with pharmacists and other health care providers to ensure safe and cost-effective medication therapy for clinic patients. 6. Contributes to the achievement of the clinical and sustainability goals of the department. 7. Provides in-service training for pharmacy personnel or other clinic personnel as related to pharmaceutical issues. 8. Provides pharmaceutical information, as needed and requested, to other healthcare providers. 9. Participates in the following committees/programs: a. Participates in quality assurance programs and projects supporting the vision and mission of the pharmacy and clinic. b. Member of the clinic provider team to discuss clinical and pharmaceutical topics affecting patient care. c. Member of various clinic-wide committees as delegated by Pharmacy Director. 10. Utilizes and assists with supervision of pharmacy technicians/assistants, technicians in training and pharmacy students/interns as appropriate with the pharmacist assuming final responsibility for work performed by staff. The pharmacist only delegates duties or activities to the staff as allowed by law and policies/procedures. Provides appropriate instructions/education for pharmacy staff. 11. Is responsible for knowledge of and compliance with CVCH health care and pharmacy department requirements for fire/safety/disasters, hazardous materials, aseptic technique, infection control, use of personal protective equipment, and confidentiality issues. 12. Assists with training and orientation of new pharmacy employees or students. 13. After competency training and assessment will be responsible for the outpatient management of anticoagulation in clinic patients via a prescriptive protocol. This management includes patient education, warfarin dose adjustment, monitoring of signs of bleeding and thrombosis, and triage of serious side effects to the appropriate medical provider.

Job Specifications

1. Education: Bachelor of Science in Pharmacy or a PharmD from an accredited school or college of pharmacy. Completion of a PGY1 Community Pharmacy or PGY2 Ambulatory Care Accredited Residency is preferred.

2. Certification/Licensure: Washington State licensed pharmacist or eligible to apply for licensure and willingness to be licensed as a Washington State pharmacist preceptor. Board of Pharmaceutical Specialties certification preferred. Basic Life Support required.

3. Experience: Experience in clinic pharmacy setting and/or ability and interest to learn. Works well with pharmacy systems. Clinically competent. Knowledge and experience of ordering, inventory and cost management activities. Experience in personnel management and/or ability and interest to learn. Two years of community pharmacy experience is preferred.

4. Language Skills: Bilingual in English/Spanish preferred.

5. Essential Technical/Motor Skills: Demonstrates competency prior to use, on all equipment listed on department-specific checklist. Must have basic computer and keyboarding skills and have the ability to enter data within company's computer system. Knowledge of computer applications and equipment related to work. Ability to input/type data, copy documents, and answer telephones.

6. Interpersonal Skills: Utilizes appropriate channels for conflict resolution. Conveys essential information to members of the healthcare team utilizing a collegial manner. Works within a team concept. Exhibits tact and understanding in all interpersonal relations. Demonstrates flexibility, adaptability, and good problem-solving skills in a variety of settings. Complies with the employee standards including dress code, policies and procedures, and scheduling. Works calmly and efficiently under pressure, handling demands from multiple sources. 7. Essential Physical Requirements: Hand, wrist and finger dexterity required to open a variety of containers and perform manipulations with needle and syringe. Hearing ability sufficient to clearly interpret phone orders and drug/health information questions. Skill required to accurately operate typewriter, calculator, adding machine, and computer equipment. Clear vision required to: precisely measure liquids and solids by sight, correctly weigh solids on a balance, accurately read and interpret orders, medical information, and journals. Communicates well both verbally and written for accurate exchange of information with physicians, nurses, health care professionals, patients and/or family, and the public. Manual dexterity and strength required to: stand and walk 8 to 12 hours a day, stretch, bend, and reach high and low frequently throughout the workday, lift boxes, freight and supplies, lift and/or carry up to 25 lbs., and push or pull equipment/supplies. Ability to work variable hours.

9. Essential Sensory Requirements: Ability to: read computer keyboard, monitor, and documents; prepare and analyze documents; read extensively; hear, recognize, and assess verbal presentations of patients; receive and convey detailed information orally, by telephone and in person. Ability to communicate clearly by speaking and hearing in person or on the phone. Smell and color perception of solutions, capsules, and tablets; vision required to precisely measure liquids, and solids; hearing capability for information and orders; accurately reads and interprets orders.

10. Exposure to Hazards: Position is required to work inside the clinic facility in offices and patient care areas. Possible exposure to blood borne pathogens and infectious disease via patient contact exposure. Chemotherapeutic agent preparation, (exposure is minimal with appropriate use of personal protective equipment).
