Details Specialty: Physical Therapist Discipline: Therapy Start Date: ASAP Duration: 13 weeks Hours: 40 hours per week Shift: 8 hours, days Employment Type: Travel Responsibilities Assess referred patients that require rehabilitation services, providing direct patient care to assess medical condition, functional capabilities, limitations, restrictions and potential for rehabilitation. Establish and administer a treatment program with specific goals according to the patient's capacity and tolerance under the direction of the Physician. Set realistic and achievable goals for patients, document and record patients' condition and educate patients and families in appropriate physical therapy method. Qualifications Must have graduated from an accredited school. Current state license as a Registered Physical Therapist / PT or proactively in the process of applying for current state licensure. Current CPR certification. Develop a plan of care for each physical therapy patient. Provide skilled physical therapy services/interventions in accordance with physician orders. Assess patient needs, plan for, evaluate and modify care to meet goals of physical therapy interventions. Collaborate with all disciplines to plan and evaluate team goals for each patient.
