A culture built around teamwork, respect, and clinical excellence.What you'll do:Perform skilled nursing visits in patients' homes.Provide wound care, medication management, and ongoing patient assessments.Educate patients and caregivers to promote independence.Communicate changes in condition with the RN Case Manager and healthcare team.Complete accurate and timely documentation.Help patients remain safe, healthy, and comfortable at home.What we're looking for:Active Minnesota Licensed Practical Nurse (LPN) license.Reliable transportation, valid driver's license, and current auto insurance.Ability to work independently and manage a daily field schedule.Previous experience in skilled nursing, rehabilitation, TCU, hospice, or home health is helpful but not required.Compassionate clinicians who enjoy building relationships with patients.Military Veterans encouraged to apply.
