Implement evidence-based PT interventions focusing on gait, balance, strength, ROM, functional mobility, and activities of daily living. Document treatment sessions clearly in the resident’s medical record and communicate progress to the interdisciplinary team. Monitor safety, apply appropriate modalities and therapeutic exercises, and adapt plans based on patient response and goals. Educate residents and families on home exercise programs, fall prevention, and transfer/ambulation strategies to support continuity of care after discharge.
