Family Practice/Primary Care Nurse Practitioner

Client seeks Nurse Practitioner or Physician Assistant for June 19-August 28 at their Corporate Health clinic in Arcadia, IN. Provider will see ages 2+ for acute episodic care, ages 5-12 for sports/camp physicals, and ages 12+ for Primary Care, acute episodic care, physicals, and wellness visits. Dispensary onsite for ages 12 and up. Hours are Tuesday 2:00pm-6:00pm, Wednesday 7:00am-11:00am, and Friday 7:00am-5:00pm wit a 60-minute lunch break. Shifts are available on Monday, Wednesday afternoon, & Thursday at sister clinic in Kokomo (30 minutes away), and Client prefers provider who can work both sites. Provider must be Board Certified and must have 2+ years of clinical experience in a Primary Care specialty to qualify for this assignment. DEA is preferred. Detailed shift needs are June 19, 23, 24, 26, & 30, July 1, 7, 8, 10, 14, 15, 17, 21, 22, 24, 28, 29, & 31, and August 4, 5, 7, 11, 12, 14, 18, 19, 21, 25, 26, & 28.
