Responsibilities
- Provide high-quality nursing care to residents in a long-term care/skilled nursing facility
- Administer medications, perform treatments, wound care, and manage chronic conditions
- Assess, plan, implement, and evaluate resident care plans in collaboration with the interdisciplinary healthcare team
- Monitor resident conditions and promptly respond to changes in status
- Supervise and support LPNs and CNAs as assigned
- Serve as charge nurse when assigned
- Maintain accurate documentation and comply with all facility, hospital, and nursing standards
- Common diagnoses: Dementia, Alzheimer's disease, diabetes, COPD, CHF, hypertension, post-acute recovery, and age-related conditions
- ALC (Awaiting Long-Term Care Placement) residents
- Typical patient ratios: 1:20-30 (may vary by facility)
- Active Registered Nurse (RN) license in New York State
- Minimum of 1 successful traveler assignment completed (first-time travelers not considered)
- BLS certification required (ACLS preferred)
- Meditech experience required
- Graduate of an accredited School of Nursing
- Strong interpersonal, organizational, and communication skills
- Ability to function effectively in a fast-paced long-term care environment
- Demonstrated leadership, delegation, and prioritization skills
