The LPN functions as part of the interdisciplinary health care team. May be required to make home visits.Provides direct and indirect nursing care individualized to the patient's needs.Collects and records relevant patient health data accuratelyEvaluates health data to identify patient problemsImplements and documents specified interventionsIdentifies deviations from expected patient outcomesAdheres to approved, research-based practice standardsPerforms and documents Point of Care (POC) testing and quality control in accordance with established laboratory policies and proceduresMaintains and documents testing reagents/supplies and equipment according to established laboratory proceduresAdvocates for patients to ensure that care preserves and protects the patient's autonomy, dignity and rights.Delivers care in a non-judgmental and non-discriminatory manner that is sensitive to patient diversityAssures that patients have informed consent signed for indicated proceduresMaintains current licensure, certification, and competencies.Provides proof of current BLS validation to supervisor.Completes required unit credentialing according to departmental policyProvides to supervisor evidence of completion all required annual training and competency validationPractices within the New York State-defined LPN scope of practice 100% of the timeAcquires and maintains current knowledge and skills in his/her area of practice.Performs other duties as requested and observed by supervisor or managerWhat we need from youEducation:Graduate of a Practical Nursing program, requiredExperience:Previous healthcare experience, preferredLicense/Certifications:Current NYS LPN License and registration as a Practical Nurse or a valid interim permit, requiredBLS certification must be obtained within 6 months of hire, requiredSkills:Clinical SkillsMulti-TaskingInteracting with PeopleAnalysis & EvaluationTake InitiativePhysical Requirements:Heavy Work - Exerting up to 100 lbs. of force occasionally, and/or up to 50 lbs of force frequently, and/or up to 20 lbs, of force constantly to move objectsOccasional climbing, crouching, grasping, kneeling, lifting, pulling, pushing, reaching, stooping, visual acuity (color, depth, perception), fit testingFrequent balancing, typing/clerical/dexterity, hearing, repetitive motion, seeing/monitor/computer use, standing, walkingHazards:Occasional burns, chemical, electrical, mechanical, radiation/radiant energyFrequent bodily fluids/bloodborne exposureWorking Conditions:Occasional noise, dust, fumes, gases, mists, odorsTravel:Occasional travel - the job may require travel from time-to-time, but not on a regular basisTotal RewardsBassett Healthcare Network's commitment to our employees includes benefit programs carefully designed with the needs and values of all our employees in mind.
