Key Responsibilities
- Evaluate and treat patients in a Skilled Nursing Facility (SNF) setting
- Develop and implement individualized physical therapy treatment plans
- Monitor and document patient progress toward therapy goals
- Collaborate with interdisciplinary care teams to coordinate patient care
- Educate patients and families on therapeutic exercises and home programs
- Active Physical Therapist (PT) license in the state of Minnesota
- Graduate of an accredited Physical Therapy program
- Experience working in a SNF or long-term care environment
- Strong clinical assessment and documentation skills
- Prior experience with geriatric patient populations
- Familiarity with skilled nursing facility documentation and compliance requirements
- Guaranteed 35-hour work week providing consistent and stable scheduling
- Opportunity to make a meaningful impact in a skilled nursing facility setting through a 13-week contract engagement
