VNA is looking for a RN Case Manager.Job Summary: The Hospice Case Manager collaborates with physicians and the interdisciplinary care team to identify, coordinate, and address the home care needs of patients, families, and caregivers. The Hospice Case Manager provides skilled nursing care and supportive services using the nursing process, with a focus on patient comfort, safety, and quality of life.They refers and coordinates other disciplines and specialty services and facilitates transitions to other levels of care or services, as needed. Manages patient care in accordance with agency standards, applicable regulations, and payer requirements. Provides education and support to patients, families, and caregivers and prepares patients and families for discharge when appropriate.Status: Full-TimeShift: Monday - Friday w/ rotating weekendsEducation/Experience/Skills/Qualifications:Active Registered Nurse license in CaliforniaMinimum of one year nursing Home Health or Hospice experience requiredKnowledge of home health, hospice, palliative care, and other agency services preferredCurrent CPR certificationAbility to work collaboratively with physicians, families, caregivers, and the interdisciplinary team membersSpecial Requirements:Current driver's licenseAuto insuranceCurrent CPR certificationReliable vehicleJoining VNA means becoming part of a trusted, mission-driven organization with more than 74 years of community-based service. The salary range provided reflects the lowest to highest amount we reasonably anticipate paying for this position at the time of posting. Compensation is influenced by labor market conditions and various factors, including but not limited to: prevailing minimum wage in the area, relevant education, experience, skills, geographic location, performance, travel requirements, sales or revenue targets, and business needs. It is not typical for a new hire to be offered the highest end of the salary range.
