Key Responsibilities of the Physical Therapist

• Perform physical therapy evaluation and treatment based on physician orders/referrals
• Conduct comprehensive evaluations using appropriate tools and procedures
• Establish measurable goals and develop treatment plans
• Regularly reassess treatment effectiveness and modify as needed
• Communicate patient needs and progress to treatment team and others involved
• Collaborate with medical providers on patient progress and discharge plans
• Recommend and assist with adaptive equipment to enhance patient function
• Maintain documentation as required by regulations and company standards
• Adhere to professional practice standards and legal requirements
• Supervise Physical Therapist Assistants and Aides in patient care activities
• Participate in staff development programs and continue clinical education

• Experience with HCHB Homecare, home exercise planning, and Oasis
