A Home Health Sales Consultant / Account Executive is a field-based growth specialist responsible for driving patient admissions by building and maintaining strong relationships with healthcare referral sources. Working as a bridge between clinical providers and the home health agency, this role focuses on generating qualified patient referrals from hospitals, physician practices, skilled nursing facilities (SNFs), assisted living communities, and discharge planners.

Key Responsibilities

Relationship Building & Territory Management

• Develop, execute, and maintain a strategic territory growth plan to achieve or exceed monthly/quarterly admission targets.
• Build and nurture high-trust relationships with key healthcare decision-makers, including physicians, case managers, hospital discharge planners, social workers, and facility administrators.
• Conduct regular in-person sales visits and clinical presentations to educate providers on home health services, clinical capabilities, and admission criteria.

Referral Generation & Account Development

• Identify new clinical accounts and prospective referral sources within the assigned geographic territory.
• Conduct clinical account analysis to uncover unmet patient care needs and align agency offerings with provider preferences.
• Monitor referral-to-admission conversion rates and implement targeted strategies to minimize patient drop-off or referral leakages.

Cross-Functional Collaboration & Market Intelligence

• Partner closely with clinical intake managers, clinical liaisons, and nursing leadership to ensure smooth patient transitions from hospital or clinic to home care.
• Gather market intelligence on competitor offerings, reimbursement trends, and local healthcare landscape shifts.
• Maintain accurate records of sales activities, account interactions, and pipeline management using a Customer Relationship Management (CRM) tool (e.g., Salesforce, PlayMaker).

Qualifications & Skill Requirements

Work Experience

• Required: 2+ years of successful direct B2B or healthcare sales experience with a documented track record of meeting or exceeding sales quotas.
• Preferred: Direct experience in home health, hospice, medical device, pharmaceutical, or post-acute healthcare sales.

Core Competencies & Skills

• Consultative Selling: Ability to conduct needs-based conversations with medical professionals and present tailored clinical solutions.
• Clinical Understanding: Basic knowledge of post-acute care models, Medicare/Medicaid regulations, and private insurance authorization processes.
• Communication: Exceptional verbal, written, and presentation skills suited for high-level healthcare professionals.
• Territory Management: Strong self-management, routing, and prioritization skills to efficiently cover a geographic territory.

Minimum Education & Credentials

• Bachelor’s degree in Business, Marketing, Healthcare Administration, or a related field (or equivalent combination of education and experience).
• Valid state driver’s license, reliable transportation, and proof of automobile insurance.
