This position involves providing comprehensive home health therapy services to adult and geriatric patients in South Marin County, including towns like Sausalito, Tiburon, Belvedere, and Mill Valley. The therapist will deliver patient-centered care, develop treatment plans, and collaborate with an interdisciplinary team to promote recovery and functional independence. Job Details Certifications, Education Requirements, Degree Requirements, Location Requirements, and Years of Experience: Valid California Physical Therapist license, BLS certification, valid driver's license and auto insurance, EPIC and Oasis experience required; 1 year of clinical experience preferred. Schedule: Days shift, working 8 hours per day, with a minimum of 40 hours per week. Additional Qualifications: Knowledge of professional practice and regulatory standards, excellent communication skills, ability to work independently in a demanding environment, and strong organizational skills. Demonstrates critical thinking and complex problem-solving abilities, with the capability to develop treatment protocols and educational materials. Responsibilities: Delivering skilled physical therapy treatments, assessing patient progress, educating patients and families, developing individualized care plans, and maintaining accurate documentation. Submission Requirement: Candidates must upload valid certifications and Licensure documentation, with references within the last year, and complete onboarding labs within 3-5 days of contract signing.
