About This Role The Registered Nurse Case Manager will support Northern Virginia by delivering coordinated, patient-centered care through in-person visits and telephonic outreach. This role is ideal for compassionate and motivated individuals looking to improve health outcomes for members. Highlights Pay : $60,522 - $129,615/year Location : Alexandria, VA Schedule : Monday–Friday, 8:00 AM – 5:00 PM EST Travel : Up to 75% of the time What You'll Do Conduct comprehensive assessments (telephonic and face-to-face) to evaluate members’ medical, behavioral, and social needs. Develop and implement personalized care plans that promote both short- and long-term wellness. Coordinate care across providers, programs, and services to ensure a seamless experience for members. Use clinical expertise and data insights to identify risks, gaps in care, and opportunities for intervention. Support members with complex or multiple conditions by applying a holistic, whole-person approach. Facilitate referrals to appropriate clinical and community resources. Participate in case conferences and collaborate with multidisciplinary teams to optimize care strategies. Review claims history and benefit eligibility to inform care planning. Engage members using strong interviewing skills to build trust and encourage active participation in their care. Ensure all case management activities comply with regulatory and organizational guidelines.
