Position Overview
The Senior Community Care (SCC) program is a longitudinal, integrated care delivery model focused on coordinating and providing comprehensive clinical care to high-risk health plan members residing in senior living and facility-based settings. As an Advanced Practice Clinician (APC), you will serve as a key member of an interdisciplinary care team, delivering high-quality, patient-centered care that improves health outcomes and enhances quality of life.
This role offers exceptional work-life balance with a flexible Monday-Friday schedule, no on-call responsibilities, no weekends, and no holiday requirements. Providers have the flexibility to adjust schedules while maintaining continuity of care for patients.

Key Responsibilities
- Conduct comprehensive health assessments for senior and high-risk patients.
- Diagnose, treat, and manage acute and chronic medical and behavioral health conditions.
- Develop and implement individualized care plans in collaboration with patients, caregivers, and interdisciplinary team members.
- Ensure accurate and complete clinical documentation, including ICD-10 coding and risk adjustment documentation.
- Participate in care coordination activities and collaborate with physicians, nurses, social workers, and other healthcare professionals.
- Conduct advanced illness management and advance care planning discussions with patients and families.
- Provide patient and caregiver education, counseling, and support regarding disease management, treatment plans, and preventive care.
- Monitor patient progress and adjust treatment plans as needed to achieve optimal outcomes.
- Maintain compliance with organizational, state, and federal healthcare regulations.
- Schedule: Monday-Friday, 8:00 AM - 5:00 PM
- No on-call requirements
- No weekends
- No holidays
- Dress Code: Medical attire
- Setting: Senior living and facility-based care environments
- Service Area: Fayetteville and Brunswick, North Carolina
- Travel During: Regular workday/shift

- Reliable transportation required
Licensure & Certification
- Active and unrestricted Nurse Practitioner or Physician Assistant license in the state of North Carolina, or ability to obtain prior to start date.
- Current DEA license with prescriptive authority.
- Ability to obtain a collaborative practice agreement if required by state regulations.
Nurse Practitioner
- Graduate of an accredited Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) program.
- Graduate of an accredited Physician Assistant program.
Nurse Practitioner
Current national certification through:
- American Academy of Nurse Practitioners (AANP), or
- American Nurses Credentialing Center (ANCC)
- Family Nurse Practitioner (FNP)
- Adult Nurse Practitioner (ANP)
- Gerontology Nurse Practitioner (GNP)
- Adult-Gerontology Acute Care Nurse Practitioner (AGACNP)
- Board Certified through the National Commission on Certification of Physician Assistants (NCCPA).
- Ability to perform physical requirements of the role, including:
- Lifting and transporting a 30-pound bag
- Navigating stairs and various residential/facility environments
- Sitting, standing, bending, and kneeling to perform patient assessments
- Access to reliable transportation
- Full COVID-19 vaccination as required by federal, state, local, and client regulations
- Ability to pass a pre-employment drug screening
- Minimum one year of clinical experience, preferably in a primary care, senior care, or long-term care setting.
- Experience caring for patients with complex medical, behavioral, social, and functional needs.
- Strong understanding of geriatrics and chronic disease management.
