The Care Navigator at SelectQuote, Inc. works from home helping patients in the Chronic Care Management program through regular calls and personalized care plans. You will educate patients, address concerns, and coordinate with PCPs, specialists, pharmacists, and community resources to improve health and reduce care gaps.
