Community Health Worker

Community Health Outreach WorkerResponsible for working closely with individuals and communities to promote and maintain good health. Responsible for providing education, support, and outreach services to community members to help them achieve their health goals. Areas could include exercise, mental health, medication management, nutrition, health care system navigation, substance use and other health behaviors. Works under the direction of a healthcare professional to provide preventive care and health education to members of the community.Essential Functions:Conduct community outreach and engage community members to understand their health needsProvide individual and group health education and coaching to community membersAssist community members in accessing and navigating the healthcare system, including providing referrals to appropriate health servicesProvide basic health screenings and assessments to identify health risks and provide appropriate interventionsAssist in the development of health promotion programs and activities to address community health needsCollect data and maintain accurate records of community health outreach activities and interventionsAttend meetings and trainings related to community health promotion and educationQualifications:Education: High School Diploma or Equivalent required or Associate's Degree Social Work preferred or Bachelor's Degree Related Field of Study preferredExperience: Experience in community outreach, health education, or related field 0-1 year preferredKnowledge, Skills and Abilities:Strong communication and interpersonal skills, with the ability to interact effectively with various populationsDemonstrated ability to work effectively and provide advocacy for all populations and communities preferredAbility to work independently and as part of a teamBasic computer skills, including Microsoft Office and database managementBilingual skills in Spanish and English RequiredAdditional Job Details (if applicable):Provide short-term intensive case management and in person support as needed to advocate for appropriate services, benefits, and programsAccompany patients to appointments (housing, court, medical, etc.) to provide support and advocacy as neededWork Location: 3297 Washington StreetScheduled Weekly Hours: 40Employee Type: RegularWork Shift: Day (United States of America)Pay Range: $23.51 - $26.58/HourlyEEO Statement:2200 The Brigham and Women's Hospital, Inc. is an Equal Opportunity Employer.
