This opportunity is in two different locations. They would like one provider for both locations.
Primary Care/Acute Care Clinic.
Ages 2 and up.
ASAP - Aug. 31 with a possible extension,
Kokomo, IN.
M - 7:00a - 4:00p
W, TH - 8:30a - 5:00p
Angelo, IN
T - 2:00p - 6:00p
F - 7:00a - 5:00p
Clean IN FNP license
2 years or more of FNP work experience
DEA
Malpractice, Lodging are provided. Mileage reimbursed.
