You will report to the associate medical director. Humana's Primary Care Organization is one of the largest and fastest-growing senior-focused, value-based care providers in the country. We are looking for a primary care physician to join our team working at CenterWell Primary Care clinics based in Apopka and surrounding offices depending on the location needs.

Responsibilities

• You will deliver comprehensive outpatient care to adult and senior patients.
• You will maintain accurate medical records and documentation.
• You will diagnose and manage moderately to complex medical conditions.
• You will coordinate referrals and collaborate with specialists.
• You will participate in clinical quality improvement projects.
• You will work with interdisciplinary teams to ensure holistic care.
• You will contribute to strategic programs and innovations in care delivery.

Required Qualifications

• MD or DO from an accredited medical school.
• Active, unrestricted medical license in the state of Florida.
• Board certification or board eligible in family medicine, internal medicine, or geriatric medicine.
• Experience in chronic condition management or managing high-acuity geriatric patients.
• Commitment to improving patient experience and outcomes.
• If selected for this role, you will be required to be screened for TB.

Must be willing to participate in limited on-call coverage by phone

Other qualifications:

• Proficiency with electronic health records (EHR) and digital documentation.
• Experience supervising advanced practice providers

Pay Range

$310,000 - $368,500 per year
