Job Summary

Responsibilities

• Provide direct patient care to clients, including assistance with daily living activities, vital signs, and basic hygiene needs
• Collaborate with our interdisciplinary team to develop and implement personalized care plans
• Maintain accurate and detailed records of patient care, including progress notes and medication management
• Ensure a safe and supportive environment for clients, including fall prevention and infection control measures
• Communicate effectively with clients, families, and healthcare professionals to ensure seamless care coordination

Job Requirements

Minimum 5 years of experience as a Certified Nursing Assistant (CNA)
High School diploma or equivalent
Certification as a Certified Nursing Assistant (CNA)
