Job Description:

We are seeking a qualified and experienced Staff Pharmacist Outpatient to provide comprehensive pharmacy services in an outpatient setting. The pharmacist will be responsible for prescription review, medication dispensing, medication reconciliation, patient education, and collaboration with prescribers and multidisciplinary healthcare teams to ensure safe and effective medication therapy.

Responsibilities:

• Work collaboratively with multidisciplinary healthcare teams to optimize drug therapy.

• Receive and review prescriptions for a full range of pharmaceuticals, including controlled substances.

• Determine the appropriate medication, strength, dosage, and quantity to be dispensed according to accepted pharmacy practices and established SOPs.

• Enter prescriptions into the outpatient pharmacy computer system.

• Review patient profiles for allergies, drug-drug interactions, drug-food interactions, and duplicate orders.

• Coordinate with prescribers to clarify medication dosing, interactions, instructions, or other prescription-related concerns.

• Prepare medications and package labels for dispensing.

• Visually inspect medications and verify the medication and printed label against the original prescription for accuracy.

• Provide patients with appropriate instructions regarding the safe and proper use of medications.

• Perform medication reconciliation for patients.

• Mentor pharmacy technicians and other pharmacy staff within the assigned area.

• Maintain accurate documentation and follow pharmacy policies, safety standards, and regulatory requirements.

• May serve as a designated Code Responder and respond to Code Blue emergencies during assigned shifts.

Requirements:

• Minimum 1 year of outpatient pharmacy experience within the past 2 years following graduation .

• Strong knowledge of outpatient pharmacy operations and prescription processing.

• Strong attention to detail and medication safety awareness.

• Excellent patient counseling and communication skills.

• Ability to collaborate effectively with prescribers, pharmacy staff, and multidisciplinary healthcare teams.

Education:

• Required:

  • Bachelor's degree in Pharmacy from a college or university accredited by the Accreditation Council for Pharmacy Education (ACPE) .

• Equivalency determination will not be accepted as a substitute for the education requirement.

License:

• Current, full, active, and unrestricted license to practice as a Pharmacist.

Certifications:

• Basic Life Support (BLS) Required

• Board Certification Not Required

Work Schedule:

• Outpatient pharmacy operations are scheduled 7 days per week, between 5:00 AM and 10:00 PM .

• Assigned shifts may range from 8 12 hours .

• Each shift includes a 30- or 60-minute unpaid lunch , depending on the section and assigned shift.

• Regular duty hours are expected to average approximately 40 hours per week .

• Pharmacists may rotate between ambulatory/outpatient and inpatient pharmacy sites based on staffing needs.

• Weekday on-call: 4:00 PM 7:00 AM the following day, Monday through Friday.

• Weekend on-call/call-back: 48-hour coverage periods on Saturday and Sunday.

• Federal holidays falling on duty days may require an additional 24-hour on-call period .

• Unplanned schedule changes may occur based on operational requirements.
