Cancellation Policy: Agency shall provide two (2) weeks' written notice of any cancellation of an assignment unless otherwise agreed upon in writing between Agency and Qualivis. Should Agency be unable to provide such two (2) weeks' cancellation notice, Agency shall pay for two (2) weeks of service at the regular hourly billing rate, up to a maximum of $2,250. Qualivis, in its sole discretion, may waive this fee if (i) Agency submits a suitable replacement Candidate within fourteen (14) days of the cancellation and (ii) such replacement Candidate starts within twenty-one (21) days of the cancellation.
