Licensed Social WorkerAs a Licensed Social Worker, you'll play a meaningful role in helping residents feel heard, supported, connected, and at home. You'll build trusted relationships with residents and their families, help identify and address psychosocial needs, and advocate for each resident's emotional and social well-being throughout their stay.From welcoming a new resident and helping families navigate the transition to skilled nursing, to providing support during changes in condition, care planning, and challenging situations, you'll be a trusted resource for residents, families, and the care team. You'll connect residents with community resources and additional services when needed, support meaningful social engagement and emotional security, and help ensure each resident's voice and individual needs remain at the center of their care.ResponsibilitiesDevelop relationships with residents and their responsible party as needed for admissions, counseling, and identifying needsWork with responsible party and resident on social interaction, reality orientation, and intellectual stimulationAssist Business Office Manager in solving financial needs and promote emotional security for the residentComplete acute care plans as needed and attend care plan meetingsAssist Administrator with resident/family/responsible party concern managementAttend committee meetings as required.Complete monthly QA reports.As needed, refer residents for vision, dental, and hearing servicesAssist with room changes by contacting responsible party and resident and completing necessary formsComplete the initial, routine, and as needed psychosocial and socialization assessment, if needed the LSW can refer resident for additional professional psychosocial services.Discuss advance care planning with responsible party upon admission and as per state regulations and complete necessary documentsMake morning rounds prior to morning meetingBe prepared to provide staff with any important department updates at the department head daily morning meeting Perform suicidal/homicidal assessments and required follow up Assist in investigations as requested by AdministratorAssist with the completion of the MDS as neededWork rotating weekend Manager on duty as scheduledQualificationsBachelor's or Master's in Social WorkLicensing RequirementsMust be a Licensed Social Worker in the state of TexasExperience RequirementsPrevious experience working in a skilled nursing facility is preferredJob InfoJob Identification 870611Job Category FacilityJob Schedule Full timeLocations 4650 S Panther Creek Dr, Spring, TX, 77381, USWork Days : Standard
