The Licensed Practical Nurse (LPN) provides direct patientcare in a community health setting under the supervision of aRegistered Nurse (RN)or licensed provider. The LPNcollaborates with the interdisciplinary care team to promote safe,efficient, and patient-centered carein accordance withstate scopeof practice and organizationalpolicies. Shift: This is a Monday-Friday position, NO weekends, NOon-call responsibilities! Pay Rate: $24-30/hour, commensurate upon experience. Performsupervised, focusedassessments,collect patient data, anddocument accurately in the electronic health record. Administer medications and immunizations, includingintramuscular(IM), subcutaneous (SQ),andoralconsistent with training and policy. The LPN Prepares andassistsproviderswith examinations andprocedures, including GYN exams and minor in-office procedures. Perform point-of-care testing, specimen collection, EKGs, andrelated clinical tasks. Provide supportinpediatric, adult, and women’s health visits. The LPN assistswith substance use disorder services, includingurine toxicology collection, and reinforcement of treatment plans asdirectedby licensed providers. Reinforce patient education based onpreviousdocumentation fromchronic disease casemanagementRNs’ care plans, includingmedications, lifestyle modifications, and preventive care instructions. Complete clinical paperwork, prior authorizations, referrals,disability/FMLA forms, and other required documentation incollaboration with providersand RNs. Conduct follow-up calls and outreach topatientsregardingappointments, andcare gaps. Conduct follow-up calls and outreach to patients regardingminimally abnormal lab results based on documented educationprovided by RNs or licensed providers. Informpatientof normal lab results tasked by licensed providers. Monitor and report changes in patient condition to the supervisingRN orlicensedprovider. Maintain exam room readiness, infection control standards, andclinical supply inventory. Participate in quality improvement initiatives and supportpopulation health efforts. Collaborate with interdisciplinary team members to ensurecoordinated, patient-centered care. Assist Medical Assistants with rooming patients when short staffed. Appropriately answer patient calls from the call center, providebasic information within scope, and ensure a warm hand-off to RNs orproviders for issues beyond LPN scope.
