What you'll do:

• Manage your own hospice caseload while serving as the primary RN for patients and families.
• Complete comprehensive nursing assessments and develop individualized plans of care.
• Provide expert pain and symptom management focused on comfort, dignity, and quality of life.
• Guide and educate patients, caregivers, and families throughout the hospice journey.
• Coordinate care with physicians, nurse practitioners, social workers, chaplains, hospice aides, therapists, and community partners.
• Provide care in patients' homes, assisted living communities, and skilled nursing facilities.
• Maintain accurate, timely EMR documentation while ensuring compliance with Medicare hospice regulations.
• Participate in a weekend and on-call rotation to provide exceptional continuity of care.

What you'll bring:

• Active Minnesota Registered Nurse (RN) license.
• Experience in hospice preferred, or a strong background in oncology, ICU, palliative care, home health, or other complex care settings.
• Strong clinical judgment and the ability to work independently.
• Excellent communication, organization, and critical thinking skills.
• A passion for relationship-centered, patient-focused care.
• Reliable transportation and willingness to travel throughout your assigned territory.
