RN - Case Management

Day shift with a schedule of 5 days per week at 8 hours per day. Job Description The Case Manager is responsible for utilization reviews, resource management, discharge planning, treatment plan management, and financial management, while also completing medical record documentation. Responsibilities Identify patient problems affecting their ability to adapt to treatment plans. Assist patients and families in utilizing hospital resources. Collaborate with the interdisciplinary team to manage clinical care plans and ensure safe discharges. Document patient care and discharge planning evaluations accurately and timely. Maintain an up-to-date community resource system for patient referrals. Ensure compliance with payor requirements for timely certifications and reimbursements. Participate in team meetings and enhance operational efficiency. Required Experience / Certifications / Licensure Current licensure in a clinical discipline per state guidelines (RN, RT, SW preferred). Associates Degree in a healthcare field (Bachelor’s degree preferred). Minimum of 2 years of experience in Case Management and Discharge Planning in a Rehabilitation Care setting. Working knowledge of the insurance industry and government reimbursement processes.
