Join Our Team! We are seeking an experienced and detail-oriented MDS Coordinator (RN or LPN) to join our skilled nursing team. This position plays a critical role in ensuring the accurate and timely completion of resident assessments, Medicare documentation, care planning, and regulatory compliance. - Coordinate and manage the Resident Assessment Instrument (RAI) process.
- Complete and submit MDS assessments accurately and within federal and state-required timeframes.
- Ensure timely completion, data entry, lock dates, and transmission of all MDS assessments.
- Monitor Medicare residents and ensure documentation accurately reflects care needs and services provided.
- Review clinical records and collaborate with interdisciplinary team members to ensure accuracy of assessments and reimbursement.
- Conduct Medicare meetings and participate in care plan meetings.
- Maintain compliance with Medicare, Medicaid, and regulatory requirements.
- Monitor RUG/PDPM reimbursement accuracy and related reporting.
- Communicate effectively with nursing, therapy, dietary, social services, physicians, and other care team members.
- Assist with staff education related to documentation, ADLs, restorative nursing, and assessment processes.
- Maintain resident confidentiality and uphold resident rights at all times.
- Current, unencumbered RN or LPN license .
- Nurse Licensure Compact (NLC) state license preferred.
- MDS experience in a Skilled Nursing Facility or Long-Term Care setting.
- MDS Certification (or eligibility to obtain certification) preferred.
- Strong knowledge of Medicare, Medicaid, PDPM, and RAI regulations.
- Excellent organizational, communication, and leadership skills.
- Proficiency with electronic medical records and MDS software.
- Ability to work independently while collaborating effectively with an interdisciplinary team.
