• Monday - Friday 8 am - 4 pm schedule
• No weekends or holidays required
• Adult panel focused on patients 55 and older
• Provider-controlled appointment scheduling
• Limited call coverage: 1 in 15 weeks
• Interdisciplinary team support with nurses and social workers
