ADON‑Reliance Home Health Caregivers, Naperville, IL 60563 Responsibilities Clinical supervision, mentorship, and performance oversight of nursing staff (RNs and LPNs) supporting an average caseload of approximately 30 pediatric patients. Oversight of patient care plans, clinical documentation, and care delivery to ensure compliance with IDPH, IDHS, Medicaid, payer requirements, and internal policies. Conducting clinical assessments, supervisory visits, and nursing competency validations. Supporting new case openings, care transitions, and coordination with intake and staffing teams to ensure timely, safe, and fully staffed service delivery. Participating in quality improvement initiatives, internal audits, corrective action plans, and regulatory surveys. Serving as a clinical escalation point for complex patient care issues and supporting clinical decision‑making. Supporting recruitment, onboarding, orientation, and ongoing education of nursing staff. Ensuring patient safety, staff competency, family satisfaction, and regulatory compliance remain at the forefront of all operations.
