Hospice Medical Social Worker (MSW) – Davie County Overview Carolina Caring is seeking a dedicated Master of Social Work (MSW) to join our team, providing vital case management and counseling services to patients, families, and caregivers during critical moments. Responsibilities Perform in-depth psychosocial assessments and develop individualized care plans in collaboration with the Interdisciplinary Team, ensuring holistic and supportive care for every patient. Provide vital case management and counseling services to patients, families, and caregivers during critical moments. Focus on serving home hospice patients in Davie County. Qualifications Master of Social Work (MSW) degree from a school of social work accredited by the Council on Social Work Education (CSWE). At least one year of social work experience in a health care setting.
