Coordinates timely care with physicians and supportive services departments. Maintains professional relationships with customers, patients, and team members. Provides direct patient care coordination within the Community Care and Transitional Care Programs. Other duties as assigned. Educates patients, families, caregivers, and community providers to safely perform care and achieve health goals. Evaluates the effectiveness of care plans and monitors patient and family responses to achieve desired outcomes. Identifies performance improvement initiatives and assists in designing or implementing programs to address needed changes. Participates in community education and outreach programs to promote health and wellness. Collaborates with Imaging Coordinator, RN Navigator, and referring providers to navigate patients through treatment planning, medical management, and surveillance. Serves as an educational resource for patients and families on treatment planning and medical management. Facilitates communication between healthcare team, patients, other departments, and outside entities by answering phone calls, responding to emails, and managing messages in the EMR and patient portal. Knowledge, Skills, and Abilities Ability to provide age-appropriate cancer education. [Required] Strong organizational, communication, and customer service skills. [Required] Knowledge of and ability to utilize in-house and external oncology resources. [Required] Self-motivated with ability to work independently.
