Registered Nurse (RN) - Home Health

Position Overview

This position requires a dedicated Registered Nurse (RN) specializing in Home Health Care. The ideal candidate will have strong case management skills and experience in using the EPIC electronic medical record system.

Schedule

Monday to Friday schedule
Rotating every fourth weekend
3 to 4 on-call shifts per month, with callbacks required between 5 PM and 8 PM

Role Responsibilities

Case management for 20-25 patients, expecting 5-6 visits per day
Utilize EPIC for documentation and patient records
Conduct comprehensive assessments and develop care plans for patients
Coordinate with healthcare team members to ensure quality patient care
Provide education and support to patients and their families

Qualifications

Experience: Minimum of 2 years of nursing experience required, preferably in home health
Certifications: CPR certification (BLS) is required
Skills: Proficiency in EPIC and knowledge of OASIS is mandatory
Preferred Skills: Experience with IV therapy, tube feeds, VAC dressings, compression wraps, port access, ostomies, chest tubes, urinary catheters, and blood draws

Business casual or scrubs (any color are acceptable)
