Licensed Practical Nurse DSD

Overview DaVita is seeking a Licensed Practical Nurse to provide compassionate care in a skilled nursing facility. You will make an exceptional difference in the lives of patients and their families dealing with end‑stage renal failure or chronic kidney disease. About the Role No dialysis experience is required. Training may take place in a facility or a training clinic other than your assigned home clinic. You may float to various clinics during and after your training. A flexible schedule is essential, and you must be able to work mornings, evenings, weekends, and holidays. Responsibilities Build meaningful and long‑term relationships with patients and their families in an intimate outpatient setting. Be a part of a team that supports and relies on each other in a positive environment. Deliver care to patients who are often dealing with multiple comorbidities, requiring unique treatment plans and the ability to leverage a broad range of nursing skills. Work with your head, heart and hands each day in a fast‑paced environment. Requirements Current LPN/LVN license in the state of practice. Current CPR certification. IV certification if required by state regulations. High school diploma or equivalent. Completion of a training program approved by the medical director and governing bodies as required by CMS guidelines before operating the water treatment system. BONENT or CHT certification preferred or required by state regulation. Minimum of one year’s experience in a hospital or clinical setting preferred. Basic computer skills and proficiency in MS Word and Outlook required; functional proficiency in DaVita computer systems within 90 days of hire required.
