Tidewell Hospice Rn Triage/Field

Since 1980, Tidewell Hospice has proudly served families across southwest Florida with compassionate, dignified care. As a member of Empath Health, we help patients with advanced illness live meaningfully while supporting their families with warmth, expertise, and respect.

Schedule: 4 nights/week 12a-8a, alternating weekends shifts. Coverage Area: Sarasota and Manatee counties; Charlotte and DeSoto service counties as needed

The RN Triage/Field:

• Visits patients and provides clinical, physical, educational and emotional support after-hours as directed by the Triage team (in the patient home, long term care facility, assisted living facility, hospice house or hospital)
• Under the direction of a licensed physician, performs patient assessment, manages discomfort and provides symptom relief using specialized nursing skills related to palliative and end-of-life care.
• Utilizes nursing interventions and teaching skills to maximize patient comfort and to enhance the quality of life to the patient/family unit.
• Attends patient deaths, provides bereavement support to the patient caregiver/ family, as indicated and performs post mortem care.
• Provides accurate and timely documentation of patient services within electronic medical record to reflect the individualized plan of care and outcomes of interventions.
• Reviews and updates the Plan of Care, physician orders, MAR/Pharmacy profile and other records to ensure patient/family needs are met.

What You'll Need

• Graduate of approved nursing program and current unrestricted Florida RN license.
• Valid and current CPR certification required.
• Minimum one year experience in an acute-care Hospital environment providing direct patient care; previous experience as a Hospice RN preferred.
• CHPN certification is strongly encouraged within three years of employment.
• BSN or MSN preferred.
• An equivalent combination of professional experience and education may be considered as meeting the educational/professional requirements.
