Cornerstone Caregiving of Wooster is expanding our 24/7 Assessment Care Team and is seeking skilled, dependable caregivers who thrive in fast-paced situations and are confident providing a high level of client care.

WHAT IS A 24/7 ASSESSMENT?

Assessment caregivers provide around-the-clock support for clients during critical transitions, changes in condition, hospital discharges, or situations where additional observation and care are needed. These cases require caregivers who can think critically, communicate effectively, and accurately report client needs and changes.

Consistency, professionalism, and attention to detail are essential, as your observations help determine ongoing care plans and client safety.

WE NEED CAREGIVERS EXPERIENCED WITH:
Personal care and total care assistance
Memory care and dementia support
Safe transfers and mobility assistance
Fall prevention and safety monitoring
Toileting, bathing, and ADLs
Meal preparation and light housekeeping
Monitoring and documenting changes in condition
Accurate and detailed care notes
Providing thorough reports to office staff and management
Working independently and exercising sound judgment
Maintaining continuity of care between shifts

REQUIREMENTS:
Valid driver's license
Reliable vehicle and current auto insurance
Ability to pass required background screening
Dependable transportation
Strong communication skills
Excellent attention to detail
Reliable attendance is essential
WHAT WE OFFER:
$16.00-$22.50/hour based on shift and applicable differentials
