Home Health Registered Nurse - Home Health

Schedule: Shift: Day 5x8-Hour (08:00 - 16:30)

Assignment Details: Contract Length: 13 weeks Guaranteed Hours: 40

Requirements: Must have both Oregon and Washington state licensure 1 year of skilled home health experience required BLS certification required OASIS experience required

Roles & Responsibilities: Provide direct patient care in home settings. Conduct patient assessments and develop individualized care plans. Educate patients and families on health management and self-care techniques. Maintain accurate and timely documentation of patient care and progress. Collaborate with interdisciplinary teams to optimize patient outcomes. Comply with facility policies and procedures.

Compliance / Notes: Traveler is expected to have their own transportation. Facility requires primary series COVID vaccination in addition to one booster. Dress code: Business casual; scrubs are allowed but not required. Orientation hours: 40 hours.
