Hybrid Case + Local Travel (ESOP)

Indiana Professional Management Group (IPMG) is hiring a Case Manager for a full-time hybrid role serving Owen County, Indiana. The position combines remote work from your home office with in-person meetings within Owen County and surrounding areas as needed, with travel up to an hour for community-based meetings.

You will educate clients about Medicaid waivers, develop individualized plans, help them access services, and facilitate regular team meetings with providers to ensure coordinated care.
