Job Description

• Conduct evaluations in inpatient and outpatient settings, tailoring individualized treatment plans for each patient
• Implement physical therapy interventions centered on enhancing mobility, strength, balance, and functional abilities
• Collaborate with fellow healthcare professionals to achieve optimal patient outcomes
• Provide education to patients and their families regarding therapeutic goals, discharge processes, and home exercise programs
• Maintain accurate documentation of evaluations, treatments, and progress in line with facility policies and regulatory standards
• Engage in care team discussions and interdisciplinary meetings to ensure comprehensive care

Education

• A doctorate in Physical Therapy (DPT) or equivalent from an accredited institution

Licenses & Certifications

• Active state licensure as a Physical Therapist
• Preferred: CPR certification
• Specialized credentials (neurological, geriatric) are beneficial but not mandatory

Skills

• Prior experience in a similar role is strongly preferred
• Capability to treat patients with intricate medical and mobility challenges
• Excellent communication and team collaboration skills
• Proficient in using electronic health record systems and clinical documentation
