Physical Therapist (PT) - per-diem- - flexible schedule: Pinnacle Health and Rehab at Sanford is searching for a caring and compassionate Physical Therapist who wants to make an important difference in the quality of life for our residents. The ideal candidate will enjoy working with the elderly as well as in a team-oriented environment. Schedule Flexible schedule, per-diem, occasional weekend day, 4-6 hours a day depending on the caseload Reports To Director of Rehabilitation General Description It is the responsibility of the Physical Therapist to provide physical therapy to residents, including evaluations, planning individual treatment programs and goals, actual resident treatments, discharge planning and other related duties. Essential Job Functions On referral, evaluate resident to determine the problems and goals of each resident in cooperation with the physician. Develop a treatment program based on initial evaluation, and with on‑going assessment, make appropriate changes in the treatment program according to the resident’s performance. Administer, schedule and supervise direct resident care, utilizing appropriate physical therapy techniques. Maintain records pertinent to resident treatment including daily progress notes, weekly summary, 14‑day physician update, discharge summary, completion of appropriate Medicare forms, resident care plan and telephone orders. Communicate with physician regarding changes or modification in treatment plan or resident status. Participate in resident care rounds, rehabilitation team conferences, conferences with physicians and members of the rehabilitation team, as necessary, regarding problems, progress and needs of the resident. Attend mandatory facility inservices. Required Job Skills Ability to work well independently and prioritize duties necessary. Effective written, verbal and interpersonal skills within an interdisciplinary model. Education/Experience Graduation from an approved program of physical therapy. Must hold a valid physical therapy license or eligibility for license in the state of Maine. Experience with geriatric population required. #J-18808-Ljbffr
